Alzheimer's RFP Information and Details
The Western Reserve Area Agency on Aging (WRAAA) Planning and Service Area (PSA) 10A located at 1700 East 13th St., Suite 114, Cleveland, OH 44114 is soliciting proposals from experienced agencies which, at the time of application, provide Adult Day Service, Respite Reimbursement and/or Institutional Care to provide respite services that assist family caregivers. Family caregivers consist of an adult family member, or another individual, who is an informal provider of in-home and community care to an older individual or to an individual with Alzheimer’s disease or a related disorder with neurological and organic brain dysfunction (related dementia) in Cuyahoga, Geauga, Lake, Lorain, and Medina Counties for SFY 2023 and 2024 Alzheimer’s Respite funds.
All documents required to submit a complete proposal will be available here, on the WRAAA Agency website beginning Wednesday June 8, 2022. One (1) Complete Proposal packet with original signatures, must be delivered to the Western Reserve Area Agency on Aging (WRAAA) AND One (1) Competitive Proposal packet with signatures submitted via Dropbox using the link below by 4:00 pm on Wednesday, July 6, 2022 in order to be considered for funding. The application documents contain instructions for submitting a proposal and outline all aspects of the proposal process.
Instructions for Dropbox Submission:
- Create a folder on your computer that includes your agency name.
- Save all application documents in that folder. Please make sure that each file includes the application item number listed on the checklist.
- Upload the entire folder containing all of the documents to the drop box.
